All you have to do is print the 2 pages out on a fairly good quality paper or card (the heavier the stock, the better), cut the design out with a scalpel and score along the dotted lines where you will fold. Then stick the tabs to the relevant positions with double sided sticky tape or glue.

This is totally free, we just like to share our designs.
